Patient centered care using transformative thought tools, the Wheel of Health and Personalized Health Planning as they relate to individual health & well-being goals. Change theory models backed by neuroscience research are used to help promote lasting change. Introductory welcome packets containing a personal health inventory and wheel of health focus areas are sent to the client for completion. The client determines the focus area they would like to transform and be willing to commit to a 3-month change management process. Meetings are based on the clients needs, but typically start with bi-weekly meetings. All sessions are with a Health & Well-Being Coach, Certified by Duke Health & Well Being. Some focus areas are mindful awareness, exercise and rest, nutritional goals, personal and professional development, relationship and communication, spirituality, mind-body connection, personal care.
